Most organisational fragility is not caused by a single catastrophic decision.
It is created gradually through hundreds of small decisions that appear sensible at the time. Every layer of approval added to reduce risk. Every conversation replaced by a process. Every opportunity to trust people replaced by another control measure.
Each decision feels logical in isolation.
Collectively they create organisations that struggle to adapt.
The irony is that these decisions are usually made with good intentions. Leaders want consistency, Governance, Efficiency and Accountability. But when control becomes the objective rather than the enabler, capability starts to decline and people stop taking ownership. Innovation slows.
Problems travel upwards because nobody feels empowered to solve them.
The organisation becomes slower precisely when speed matters most. We are seeing this challenge across sectors and arguable AI is both magnifying and masking the problem. Businesses are facing rapid technological change whilst public services are operating under increasing pressure. Institutions attempting to manage uncertainty through additional layers of process.
The answer is not less accountability. It’s better leadership.
Leadership that creates clarity without bureaucracy. Trust without naivety. Accountability without fear.
The future won’t belong to organisations that can predict every challenge. It will belong to organisations capable of responding effectively when predictions fail in a VUCA environment.
That capability starts with the decisions leaders make today.
